Suggested Gauge:

Knit: 17 sts x 23 rows = 4" on size US 8 (5 mm)

Crochet: 13 sts x 17 rows = 4" on size US J-10 (6 mm)

Machine wash - warm, do not bleach, tumble dry - medium heat, iron - low heat, dryclean - any solvent.
